Background. Benzene is a known human carcinogen. Human exposure to benzene can be assessed by measuring trans, trans-muconic acid (MUCA) in urine. Golestan Province in northeastern Iran has been reported to have high incidence of esophageal cancer linked to the use of tobacco products. This manuscript evaluates the urinary MUCA concentrations among the participants of the Golestan Cohort Study (GCS).Methods. We analyzed MUCA concentration in 177 GCS participants' urine samples and performed nonparametric pairwise multiple comparisons to determine statistically significant difference among six different product use groups. Mixed effects model was fitted on 22 participants who exclusively smoked cigarette and 51 participants who were classified as nonusers. The urinary MUCA data were collected at the baseline and approximately five years later, and intraclass correlation coefficient (ICC) was calculated from the model.Results. Compared with nonusers, tobacco smoking was associated with higher urinary MUCA concentrations. Based on the nonparametric test of pairwise multiple comparisons, MUCA concentrations among participants who smoked combusted tobacco products were statistically significantly higher compared to nonusers. Urinary MUCA collected five years apart from the same individuals showed moderate reliability (ICC = 0.41), which was expected given the relatively short half-life (∼6 h) of MUCA.Conclusion. Our study revealed that tobacco smoke was positively associated with increased levels of urinary MUCA concentration, indicating that it is a significant source of benzene exposure among GCS participants.

Resumo Introdução O metabólito do benzeno ácido trans, trans-mucônico (t, t-MA) urinário é considerado bom bioindicador de exposição a níveis de benzeno no ar superiores a 0,50 ppm. Objetivo Determinar níveis urinários de ácido trans, trans-mucônico (t, t-MA) e investigar relação com potenciais fontes de exposição ao benzeno e associação com parâmetros hematológicos. Método Estudo seccional, com 150 moradores de Açailândia, Maranhão, Brasil. Utilizou-se questionário estruturado para caracterização sociodemográfica, ocupacional, dentre outras. Coletaram-se amostras de urina e sangue para determinar níveis de t, t-MA e realização de hemograma. As associações foram exploradas mediante regressão logística e linear múltipla. Resultados O t, t-MA foi detectado em 27% dos participantes, sendo a média da concentração 0,15 mg/g de creatinina. As alterações hematológicas mais frequentes foram bastonetes baixos (41%), eosinofilia (33%) e níveis reduzidos de hemoglobina (19%). Não encontrou-se correlação estatisticamente significativa entre t, t-MA e parâmetros do hemograma. Os fatores que influenciaram de forma significativa a excreção de t, t-MA foram cor da pele, prática regular de atividades de lazer com exposição a solventes orgânicos e consumo de refrigerante e refresco nas últimas 24 horas. Conclusão A população parece não estar exposta a concentrações elevadas de benzeno. Entretanto, o perfil hematológico revelou presença de alterações bioquímicas compatíveis com anemia e comprometimento imunológico.


Abstract Background The urine benzene metabolite trans,trans-muconic (t,t-MA) is considered a good bioindicator of exposure to benzene levels in the air above 0.50 ppm. Objective Determine urinary levels of trans, trans-muconic acid (t,t-MA) and investigate the relationship with potential sources of exposure to benzene and its association with hematological parameters. Method A sectional study was carried out with 150 residents of Açailândia, Maranhão, Brazil. A structured questionnaire was used for sociodemographic and occupational characterization, among others. Urine and blood samples were collected to determine t,t-MA levels and blood counts. Associations were explored using multiple linear and logistic regressions. Results The t,t-MA was detected in 27% of the participants, with an average concentration of 0.15 mg/g creatinine. The most frequent hematological changes were low rods (41%), eosinophilia (33%), and reduced hemoglobin levels (19%). No statistically significant correlation was found between t,t-MA and blood count parameters. The factors that significantly influenced the excretion of t, t-MA were skin color, regular practice of leisure activities with exposure to organic solvents, and consumption of soda and soft drinks in the last 24 hours. Conclusion The population does not appear to be exposed to high concentrations of benzene. However, the hematological profile revealed the presence of biochemical changes compatible with anemia and immunological impairment.

Background: The Thai government has been developing its Eastern Economic Corridor (EEC), which spans three provinces, with the aim of improving connections with other Asian nations. Since this strategic development, the number of trucks, private car, and passenger car registrations have continued to grow, with a corresponding increase in related to benzene, toluene, ethylbenzene, and xylene (BTEX). Objectives: This study aims to compare the levels of trans, trans-muconic acid (t, t MA); toluene (TU); mandelic acid (MA); and methyl hippuric acid (MHA) in the urine of gas station employees, considering demographic and occupational factors. Material and methods: These employees worked either near or away from the fuel dispenser, and there 100 people in each group. Data were collected using interviews and testing environmental air and urine samples for benzene, toluene, ethyl benzene, and xylene (BTEX). Results: The results showed that BTEX concentrations were just detectable in all 200 cases (100%). The mean (±SD) urine level of t, t MA was 449.28 (±213.32) µg/g creatinine, while the median (min-max) was 428.23 (95.58-1202.56) µg/g creatinine. The mean TU was 0.011 (0.001) mg/L, while the median (min-max) was 0.011 (0.010-0.013) mg/L. MA levels were higher inside the pollution control zone than they were outside the zone (p=.009). Employees who practiced poor personal hygiene had relatively high urinary toluene and MHA levels (p=.009) and those who did not wear personal protective equipment (PPE) had relatively high MA levels (p=.040). Conclusion: The results of this study revealed statistically significant biomarkers influencing the levels of t, t MA; TU; MA; and MHA in urine. The recommendations of this study are that employers should provide their employees with suitable PPE, check regularly to ensure that it is worn, and strongly encourage employees to take care of their sanitation. Employees should take appropriate breaks and days off to minimize their exposure to BTEX.

BACKGROUND: Benzene is a ubiquitous air pollutant that is well known to cause hematopoietic effects in humans including leukemia. Recently, several studies have discussed its non-carcinogenic effects such as diabetes. This study aimed to investigate the association between diabetes and urinary trans,trans-muconic acid (t,t-MA), one of benzene metabolite, using adult data from Korean National Environmental Health Survey (KoNEHS) cycle 3 (2015-2017). METHODS: This study analyzed 3,777 adults (1,645 men and 2,132 women) from the KoNEHS cycle 3 (2015-2017). The distribution and fraction of each independent variable were presented separately according to the urinary benzene metabolite levels (t,t-MA quartiles) and diabetes to determine the general characteristics of the subjects. Odds ratios (ORs) were calculated using logistic regression after stratification by gender and smoking status to identify the association between urinary t,t-MA and diabetes. RESULTS: Compared with the first quartile (reference), the risk of diabetes significantly increased above the 4th (1.834 [1.107-3.039]) quartile in men and above the 3rd (1.826 [1.095-3.044]) and 4th (2.243 [1.332-3.776]) quartiles in women after adjustment. Stratified analysis based on smoking revealed that the ORs for the 3rd (1.847 [1.146-2.976]) and 4th (1.862 [1.136-3.052]) quartiles in non-smokers and those for the 2nd (1.721 [1.046-2.832]), 3rd (1.797 [1.059-3.050]), and 4th (2.546 [1.509-4.293]) quartiles in smokers were significantly higher. CONCLUSIONS: We confirmed that urinary t,t-MA is significantly associated with diabetes regardless of gender and smoking status. And further studies are necessary to access the clinical impacts of this findings.

In the current study, a novel technique for extraction and determination of trans,trans-muconic acid, hippuric acid, and mandelic acid was developed by means of ion-pair-based hollow fiber liquid-phase microextraction in the three-phase mode. Important factors affecting the extraction efficiency of the method were investigated and optimized. These metabolites were extracted from 10 mL of the source phase into a supported liquid membrane containing 1-octanol and 10% w/v of Aliquat 336 as the ionic carrier followed by high-performance liquid chromatography analysis. The organic phase immobilized in the pores of a hollow fiber was back-extracted into 24 µL of a solution containing 3.0 mol/L sodium chloride placed inside the lumen of the fiber. A very high preconcentration of 212- to 440-fold, limit of detection of 0.1-7 µg/L, and relative recovery of 87-95% were obtained under the optimized conditions of this method. The relative standard deviation values for within-day and between-day precisions were calculated at 2.9-8.5 and 4.3-11.2%, respectively. The method was successfully applied to urine samples from volunteers at different work environments. The results demonstrated that the method can be used as a sensitive and effective technique for the determination of the metabolites in urine.

Resumen Objetivo: Evaluar niveles de ácido trans, transmucónico (AttM) y su relación con parámetros hematimétricos y hepáticos en trabajadores expuestos a benceno en Valencia (Venezuela). Sujetos y métodos: Se desarrolló una investigación descriptiva, correlational, con diseño no experimental, de campo y transversal. Se analizaron muestras de 2 grupos de trabajadores expuestos a benceno: 30 trabajadores de una planta de empaques (TPE), 18 trabajadores de estaciones de servicio (TES) y un grupo control (GC) de 22 individuos. La medición del AttM se realizó en muestras de orina recolectadas al final de la jornada laboral, y fueron analizadas por cromatografía líquida de alta resolución con detección ultravioleta (HPLC-UV).Se determinó la biometría hemática y transaminasas (TGO y TGP). Se utilizó estadística no paramétrica para analizar los resultados. Resultados: Al comparar los valores de AttM se observaron diferencias significativas entre los grupos (TPE: 2,47; TES: 0,94; GC: 0,34 mg.g -1 creat); el hábito tabáquico no representó un factor influyente para los resultados. Los parámetros hematológicos y transaminasas para todos los grupos se observaron dentro de los valores de referencia; el contaje de plaquetas mostró correlación significativa (p= 0,033) con el biomarcador en los grupos expuestos. Conclusión: el AttM permitió evaluar la exposición ocupacional al benceno en los grupos estudiados, y presentó correlación significativa con el contaje de plaquetas, sin que este parámetro hematológico se observe alterado en comparación con los valores de referencia.


Abstract Objective: to evaluate trans, trans - muconic acid levels (ttMA) and its relationship with hematimetric and liver parameters in workers exposed to benzene in Valencia (Venezuela). Subjects and methods: A descriptive and correlational research, no experimental design, field and cross, was developed. Samples of 2 groups of workers exposed to benzene were analyzed: 30 workers at a packaging plant (PPW) and 18 employees, gas stations attendants (GSA), besides, 22 individuals in a control group. ttMA measurement was performed on urines samples collected at the end of the workday and were analyzed by high performance liquid chromatography with ultraviolet detection (HPLC-UV). Hematological and hepatics parameters (GOTand GPT) were determined in blood samples blood. Non parametric statistics were used to analyze the results Results: When comparing ttMA values, significant differences between groups were observed (PPW: 2,47; GSA 0,94; CG: 0,34 mg.g -1 creat); Smoking status not represent influential factor for the results. Hematological parameters and transaminases for all groups were inside reference values; platelet count showed significant correlation (p= 0,033) with biomarker in exposed groups. Conclusion: ttMA allowed to evaluate occupational exposure to benzene in the groups studied. It was observed significant correlation with platelet count, without alteration in the hematological parameter when compared with the reference value.

trans,trans-Muconic acid (tt-MA) has been introduced as biological exposure index for workers exposed to benzene. In the present study, for the first time, a novel and easy-to-use analytical method based on molecularly imprinted polymer (MIP) in microextraction by packed sorbent (MEPS), followed by high-performance liquid chromatography with ultra-violet detection (MIMEPS-HPLC-UV) was developed to determine tt-MA in urine samples. Fourier transform infrared spectroscopy was used for MIP characterization. The imprinting effect on the MIP was also evaluated. Furthermore, various parameters that affect the MEPS performance were investigated, including extraction cycles, sample volume, as well as the type and volume of washing and elution solvents. The optimised procedure was selective, sensitive, rapid and both user- and environmentally-friendly. A seven-point calibration curve was obtained in the range of 0.015-2µg/mL (R2=0.9986). The extraction recovery was >89.8%. The limit of detection and limit of quantitation for tt-MA were 0.015 and 0.05µg/mL, respectively. The within- and between-day precisions, given as percent relative standard deviation, were in the range of 3.4-4.7% and 3.5-6.6%, respectively. Accuracy was -8.4 to -10.2% in urine samples. The developed method is suggested as an alternative to existing conventional SPE methods for biomonitoring of benzene-exposed subjects.

Its urinary metabolite trans, trans-muconic acid (tt-MA) has been introduced by some environmental and occupational health regulatory associations as a biological index for the assessment of benzene exposure; however, recently, doubts have been raised about the specificity of tt-MA for low-level benzene exposures. In the present study, we investigated the association between urinary levels of tt-MA and inhalational exposure to benzene in different exposure groups. METHODS: Benzene exposure was assessed by personal air sampling. Collected benzene on charcoal tube was extracted by carbon disulfide and determined by a gas chromatograph (gas chromatography with a flame ionization detector). Urinary tt-MA was extracted by a strong anion-exchange column and determined with high-performance liquid chromatography-UV. RESULTS: Urinary levels of tt-MA in intensive benzene exposure groups (chemical workers and police officers) were significantly higher than other groups (urban and rural residents), but its levels in the last two groups with significant different exposure levels (mean = 0.081 ppm and 0.019 ppm, respectively) showed no significant difference (mean = 388 µg/g creatinine and 282 µg/g, respectively; p < 0.05). Before work shift, urine samples of workers and police officers showed a high amount of tt-MA and its levels in rural residents' samples were not zero. CONCLUSION: Our results suggest that tt-MA may not be a reliable biomarker for monitoring low-level (below 0.5 ppm) benzene exposures.

This study aimed to determine t,t-muconic acid (t,t-MA; exposure biomarker for benzene) and hippuric acid (HA; exposure biomarker for toluene) concentrations in the urine of women living in Mexico. In a cross-sectional study, apparently healthy women (n = 104) were voluntarily recruited from localities with a high risk of air pollution; t,t-MA and HA in urine were quantified using a high-performance liquid chromatography (HPLC) technique. Mean urinary levels of t,t-MA ranged from 680 to 1,310 µg/g creatinine. Mean values of HA ranged from 0.38 to 0.87 g/g creatinine. In conclusion, compared to data recently reported in literature, we found high urinary levels of t,t-MA and HA in assessed women participating in this study. We therefore deem the implementation of a strategy aimed at the reduction of exposure as a necessary measure for the evaluated communities.

Resumo Introdução: frentistas e analistas de combustíveis estão expostos a vários compostos orgânicos voláteis presentes na gasolina, incluindo benzeno, que se destaca por sua importância toxicológica. Objetivo: avaliar a exposição ocupacional ao benzeno na gasolina, utilizando o Teste Cometa como biomarcador de genotoxicidade em comparação ao ácido trans,trans-mucônico urinário (AttM) como biomarcador de exposição ao benzeno. Métodos: estudo de corte transversal com análises de biomarcadores de exposição e de genotoxicidade em grupo de expostos ocupacionalmente ao benzeno através da gasolina e grupo controle. Resultados: o Teste Cometa mostrou uma média (desvio padrão) de índice de dano, expresso em unidades arbitrárias, no grupo exposto de 28,4 (10,1) significativamente mais elevado do que no grupo não-exposto, 18,4 (10,1). O AttM urinário, em mg/g de creatinina, foi significativamente maior no grupo exposto, 1,13 (0,45), em relação ao grupo não exposto, 0,44 (0,33). Os dois biomarcadores apresentaram boa correlação linear (r=0,81; p<0,05), indicando uma forte associação entre o biomarcador de exposição e o biomarcador de efeito. Conclusão: os resultados sugerem que uma maior exposição ocupacional ao benzeno está associada a um risco aumentado de dano genotóxico entre indivíduos expostos à gasolina.


Abstract Introduction: gas-station attendants and fuel analysts are exposed to various volatile organic compounds found in gasoline, including benzene, which stands out because of its toxicological significance. Objective: to assess occupational exposure to benzene using the comet assay as a biomarker of genotoxicity in comparison to the urinary trans, trans muconic acid (ttMA) as benzene exposure biomarker. Methods: cross-sectional study using biomarkers of exposure and genotoxicity analyses in a group of workers occupationally exposed to benzene from gasoline compared with a control group. Results: the comet assay results showed that the mean (standard deviation) damage index, in arbitrary units, in the exposed group, 28.4 (10.1), was significantly higher than in the non-exposed group, 18.4 (10.1). The mean value of urinary ttMA was significantly higher in the exposed group, 1.13 (0.45), compared to the non-exposed group, 0.44 (0.33). Both biomarkers showed a linear correlation r = 0.81 (p<0.05) indicating a strong association between the exposure biomarker and the biomarker effect. Conclusion: the results suggest that a greater occupational exposure to benzene is associated to an increased risk of genotoxic damage among individuals exposed to gasoline.

Resumo Introdução: o benzeno é uma substância de reconhecida toxicidade e sua biomonitorização torna-se fundamental para a prevenção de danos à saúde humana, principalmente em situações de exposição ocupacional. Dentre os biomarcadores de exposição, o ácido S-fenilmercaptúrico é considerado o único específico, mas, devido a suas baixas concentrações na urina, é requerido o uso de técnicas analíticas sensíveis capazes de quantificar traços. Objetivo: revisar metodologias baseadas na cromatografia e na espectrometria de massas para a determinação do ácido S-fenilmercaptúrico. Método: revisão da literatura sobre a determinação do ácido S-fenilmercaptúrico urinário por técnicas de cromatografia e espectrometria de massas, nas principais bases de dados científicas, considerando o período entre 1951 e 2015. Resultados: 120 documentos serviram como base teórica para a construção desta revisão. A técnica analítica mais empregada foi o acoplamento da cromatografia a líquido com a espectrometria de massas. Contudo, os métodos diferem quanto ao preparo das amostras. Conclusão: o alto custo de aquisição e a manutenção de equipamentos são fatores limitantes para a difusão dos sistemas de cromatografia e espectrometria de massas. No entanto, sua elevada sensibilidade e seletividade faz com que essas técnicas, acopladas, possibilitem elucidar situações de exposição ocupacional e ambiental a poluentes, como o benzeno.


Abstract Introduction: benzene is a substance of recognized toxicity and its biomonitoring of exposure becomes critical for preventing health damages, especially in occupational exposure situations. Among all the biomarkers of exposure, S-phenylmercapturic acid is considered the only specific one, but due to its low concentrations in urine, acute analytical techniques capable of quantifying traces must be used. Objective: to review methodologies based on chromatography and mass spectrometry for determination of S-phenylmercapturic acid. Method: literature review on the determination of urinary S-phenylmercapturic acid by chromatographic techniques and mass spectrometry in the main scientific databases, considering the period between 1951 and 2015. Results: 120 documents served as theoretical basis for the construction of this review. The analytical technique that was most applied was the coupling of liquid chromatography with mass spectrometry. However, the methods differ according to the preparation of samples. Conclusion: the high cost of equipment acquisition and maintenance are limiting factors for the diffusion of chromatography systems and mass spectrometry. Nevertheless, its high sensitivity and selectivity enable these coupled techniques to elucidate situations of occupational and environmental exposure to pollutants, such as benzene.

For the first time, hollow-fiber liquid-phase microextraction combined with high-performance liquid chromatography-ultraviolet was used to extract trans,trans-muconic acid, in urine samples of workers who had been exposed to benzene. The parameters affecting the metabolite extraction were optimized as follows: the volume of sample solution was 11 mL with pH 2, liquid membrane containing dihexyl ether as the supporter, 15% (w/v) of trioctylphosphine oxide as the carrier, the time of extraction was 120 minutes, and stirring rate was 500 rpm. Organic phase impregnated in the pores of a hollow fiber was extracted into 24 µL solution of 0.05 mol L(-1) Na2CO3 located inside the lumen of the fiber. Under optimized conditions, a high enrichment factor of 153-182 folds, relative recovery of 83%-92%, and detection limit of 0.001 µg mL(-1) were obtained. The method was successfully applied to the analysis of ttMA in real urine samples.

INTRODUCTION: Traffic-related air pollution has been shown to induce neurotoxicity in rodents. Several recent epidemiological studies reported negative associations between residential outdoor air pollution and neurobehavioral performance. We investigated in a population of non-smoker adolescents the associations between the urinary concentration of trans, trans-muconic acid (t,t-MA-U), a metabolite of benzene and used as proxy-biomarker of traffic exposure, and two neurobehavioral domains, i.e. sustained attention and short-term memory. METHODS: In the framework of an environmental health surveillance study in Flanders (Belgium), we examined between 2008 and 2014 grade nine high school students (n=895). We used reaction time, number of omission errors, and number of commission errors in the Continuous Performance Test to evaluate sustained attention, and for the evaluation of short-term memory we used maximum digit span forward and backward of the Digit Span Test. We measured blood lead (PbB) to assess the independent effect of t,t-MA-U on neurobehavioral outcomes. RESULTS: This neurobehavioral examination study showed that a ten-fold increase in t,t-MA-U was associated with a 0.14 SD lower sustained attention (95% Confidence Interval: -0.26 to -0.019; p=0.02) and a 0.17 SD diminished short-term memory (95% CI: -0.31 to -0.030; p=0.02). For the same increment in t,t-MA-U, the Continuous Performance Test showed a 12.2ms higher mean reaction time (95% CI: 4.86-19.5; p=0.001) and 0.51 more numbers of errors of omission (95% CI: 0.057-0.97; p=0.028), while no significant association was found with errors of commission. For the Digit Span Tests, the maximum digit span forward was associated with a 0.20 lower number of digits (95% CI: -0.38 to -0.026; p=0.025) and maximum digit span backward with -0.15 digits (95% CI: -0.32 to 0.022; p=0.088). These associations were independent of PbB, parental education and other important covariates including gender, age, passive smoking, ethnicity, urinary creatinine, time of the day, and examination day of the week. For PbB, an independent association was only found with mean reaction time of the Continuous Performance Test (19.1ms, 95% CI: 2.43-35.8; p=0.025). CONCLUSIONS: In adolescents, a ten-fold increase in the concentration of t,t-MA-U, used as a proxy-biomarker for traffic-related exposure, was associated with a significant deficit in sustained attention and short-term memory. The public health implications of this finding cannot be overlooked as the effect-size for these neurobehavioral domains was about 40% of the effect-size of parental education.

RESUMEN

OBJECTIVES: Automobile exhaust gases contain benzene and toluene, which are excreted in human urine as trans,trans-muconic acid and hippuric acid, respectively. Sorbic acid and benzoic acid, used as food preservatives, are also metabolized into trans,trans-muconic acid and hippuric acid in the human body. The purpose of this study is to estimate the level of benzene and toluene exposure according to the commuting mode and duration, residential environment, and preservative-added foods intake in university students who are not occupationally exposed to benzene or toluene. METHODS: Spot urine samples were collected from 211 university students who had no occupational exposure to volatile organic compounds. Information about their smoking history, residence type, traffic environments, commuting mode and duration, and their intake of bottled or canned food or beverages was gathered through a self-administered questionnaire. Urinary concentrations of trans,trans-muconic acid and hippuric acid were measured and statistically correlated to the individual's life style factors. RESULTS: There was no significant difference in the urinary concentrations of trans,trans-muconic acid or hippuric acid according to the smoking history. Mean urinary concentration of hippuric acid in females was higher than that found in males. Subjects living in districts with a population density of 1,000~4,999 people/km2 showed the highest urinary hippuric acid level; individuals living in cities of 5,000~9,999 people/km2 population density were next. The mean urinary trans,trans-muconic acid concentration was higher in students who were living where traffic jams are common compared to those who did not, and in subjects whose residence were within 149 m from a 4-lane road compared to those whose residence was not. However, neither mode nor duration of the commute showed any effect on the urinary trans,trans-muconic acid or hippuric acid concentrations of the students. Urinary hippuric acid levels increased when consuming canned fruit or canned coffee, and urinary trans,trans-muconic acid levels increased when consuming fruit juices or pickled radishes. CONCLUSIONS: The inhalation of vehicle exhaust and the ingestion of canned or pickled food may increase urinary hippuric acid and trans,trans-muconic acid levels in individuals who are not occupationally exposed to benzene or toluene.
